Why is it that no matter how much time you spend proofing and rereading and no matter how satisfied you are with your work, there is always some small improvement that is invisible until immediately after you push the 'submit' button. Or am I the only one?

Try taking an 8 week holiday from your writing and then coming back to it. You'll notice more errors / clumsy writing than when you're in it day to day.
Happens to me nearly every time.
Something I just stumbled upon: if I proofread a story backward, one paragraph at a time--the last paragraph, then the one before that, then the one before that--I can see the story with fresher eyes, and catch errors and come up with solutions I might not otherwise see.
