In no particular order:
Spiders
Something piercing my eye
Solitude
Never getting to appologise to all that deserve it before I die
Oh and lastly not being able to cook a decent souffle - hate when they don't rise
Loss of a child would be first. Heights would be next.
Neglect and social indifference, one of the most damaging things that can happen to a person psychologically
I think death is a big fear too, not how i die, but death in general.
Oh and the giant nicotine patches that chase me down the street in my dreams (joke) lawl
Death, fear of the unknown, being alone and away from those I love.
I have to agree with KikiKyan - Being alone or the loss of one of my parents/sisters/brother/nephews/nieces.
Death is a big one but as I get older and closer to it I try to accept it rather than dwell on it. I would be more frightened of becoming mentally or physically disabled. I watched them hoisting my girlfriend around who had Multiple Sclerosis and my father who couldn't walk after a heart operation. They both willed themselves to death. When it comes to a revulsion type fear though poisonous snakes are very high on my list and I have met a few.

Fear of the unknown and being alone.
Losing traction on the interstate again.
Deep ocean swimming alone, due to the not knowing what is just below me.
But in my sleep it is heights, which strangely enough does not bother me in real life.
Heights is a big one then being alone and death and dying to young
heights. and when i use to be a bus driver hitting a motorcyclist. (thank god i never did because it would have been game over.)
