I've heard it called a few things. Spider-gag, I think, is the most popular name. It's also referred to as an open-mouth gag or a ring gag.

Ok mystery solved (sort of!) According to the website http://www.extremerestraints.com they call it a Whitehead Ratcheting Mouth Gag. Whether that it the technical name for it (in the dental field) or not, I don't know. But that's what I will go with!
