From how I understand it, there isn't necessarily increased blood flow, but that the smooth muscle of the penis constricts which reduces the ability of blood to leave the corpora cavernosa, therefore increasing the pressure and causing erection. Pelvic floor muscles also constrict, increasing pressure and erection. I admit it's been a few years since I did reproductive health and I deal predominantly with vaginas and babies so I could be wrong.
I'm not sure if your second part was tongue in cheek or not, but, blood flow to your whole body is approximately 7% of body weight so even if one penis is double the size of another, when you think about it in relation to the entire body, it's still going to be a negligible quantity.

@puddleduck My legs (thigh part) tend to go to sleep if we are standing while doing it. If I'm laying down and I'm keeping myself from cumming the tip of my nose will go to sleep and then my cheeks, then the forehead. When I was young this type of thing never happened.
