I queried this with the development and they are investigating. I tried various tests earlier, removing a friend, declining a friend request, member deletes etc. etc. and could not reproduce that scenario. The fact that the development team are going to investigate implies its a bug.

I did deny a very random request, but that was more than a week ago. This showed up yesterday and is gone today.
