Is there a difference? If so, what is it?
No difference that I know of People use them as meaning the same thing
In my opinion,,transgender would cover the entire spectrum of the topic, from crossdressing to actual gender reassignment surgery. Transexual,, to me,,, better describes the latter,,, actual changing of gender through surgery.

Yup.

Transexualism is technically a mental disorder in the sense it has to be diagnosed by a shrink before it goes any further. It's the only way to get the hormones or surgeries

Gosh, this was an op from what seems like a lifetime ago and I've learned and researched so much since I joined Lush.
I, too, was under the misunderstanding that sexuality and gender were interchangeable but they are seperate entities.
So now I know transexuals and transgenders are two seperate groups and crossdressing is seperate again.
Although, I do not like labels, I believe they are the same
I'm 50 and still trying to figure out what I really am. A label won't really help. I know there are medical, psychological and legal reasons for the the labels, but it seems to be a constantly evolving discussion.

Transsexual is a subset of Transgender. It is characterized by a congenital neurological disorder. It is an intersex condition naturally occurring at birth such as a female brain in a male body or male brain in a female body (Harry Benjamin's Syndrome). The condition almost always requires medical intervention to correct it, which typically includes Hormonal Replacement Therapy (HRT) and Genital Reconstructive Surgery (GRS). The medical treatment is designed to make their bodies match their psychological state (i.e., their inner gender), enabling them to assume a gender appropriate role in society. The key thing to note is that Transsexuals are born that way.

Transgender, on the other hand, is an umbrella term used to cover anyone whose gender identity, gender expression, and/or gender role doesn’t conform to the sex they were assigned at birth. Such umbrella term includes sometimes-overlapping categories such as transsexual, cross-dresser (transvestite), genderqueer, androgyne, and bigender. It usually excludes transvestic fetishists (because it is considered to be a paraphilia rather than gender identification), and drag kings and drag queens, who are performers who cross-dress for the purpose of entertaining. But it is not uncommon to see the term Transgender used to refer to drag kings / drag queens.
