Experience with orchidectomy

A gay friend is considering elective orchidectomy, testicle removal. He feels it will suit his relationship and sexuality emotionally, physically and hormonally. I am so worried for him but want to support him too. Do you know anyone who has been through this and how it affected him? What are the emotional changes? What are unexpected things i should know about?
I don't have personal experience, but know of someone who did. First off, it is a treatment for Prostate Cancer, as prostate cancer typically relies on testosterone. Generally it is not elective unless transitioning from male to female (MTF). These are the most common side effects according to my friend that had it. Besides permanently removing the testicles and becoming sterile, it is likely that sex drive will decrease as testosterone is no longer created at normal levels for a male and his sex drive. As the level of testosterone has decreased, so with the ratio of testosterone to estrogen. This means it is possible to lose muscle mass and strength, have weight gain, enlargement of breasts, future osteoporosis, lack of getting an erection or dysfunction, and possible hot flashes. Hard to say which of these someone would get as everyone is different.

I also have a friend in the process of transitioning from MTF, but she is choosing the HRT (Hormone Replacement Therapy). In essence it is similar as one pill blocks the testicles from making testosterone, and will likely cause sterilization over time. The other pill adds estrogen/progestogen. Same general side effects, except the addition of the estrogen will also cause emotional changes, and increased tendency to grow breasts and enlarge the butt area.

Bottom line, doing it shouldn't be taken lightly. If elective, a lot of thought and possibly counseling should go with it as you can never reverse it.
